Description: This photo was taken in a grocery store. During the initial inspection, I noticed the floor drain with some standing black sewage water. It was the only drain in the establishment like this, and after exacerbating the plumbing with draining and running water, no sewage backups were noticed. During the reinspection, I I pulled back the cardboard to see maggots moving and crawling out of the drain. Oct 03, 2017

Description: This was taken during a complaint inspection about mice in an establishment. I found no evidence of mice themselves or activity, but was then presented with 2 plates (including the one in this photo). It was determined that this was an intentional contamination event. The mice were field mice that had been frozen. They were placed on the plates by an employee during transport from the kitchen to the banquet room.Oct 03, 2017
